Tryouts

Tuesday & Wednesday:

Freshman: 5:00-6:15, JV: 6:15-7:30, Varsity: 7:30-9:00

Required Forms:

I have attached forms that are REQUIRED from ALL athletes before the season starts. I am in the building during the day, so these forms could be emailed, or dropped off to the school. If forms are not submitted by the first day of tryouts, they cannot tryout until I have received them.

The following forms that will need to be submitted:

Safe Sports Network - Authorization and Concussion Agreement

Safe Sports Network - Consent for Care and Treatment

Trinity High School - Concussion Agreement

Trinity High School - Code of Conduct

Trinity High School - Permission to Participate

Updated Physical (I need a copy of the most recent physical they have had within the last year. Please email me if you have questions on whether or not your child is up to date.)

**These forms cover each sports season of the current school year, so they only need to be filled out once.

Concussion Testing

Who: Freshman, Juniors, New Students and Transfers. If baselines are not completed by the first day of tryouts they cannot tryout.

What: C3 Logix is a concussion testing platform used to assess an athlete’s memory, concentration, and balance. This test provides baseline data for each athlete, which can then be used as a comparison if a head injury is sustained in the future.

When: During study or after school.

Testing is required.

**Baselines are completed once every 2 years.
